Modify Interactive PDF Form -- 2
Budget: ₹600 – ₹1,500 INR
I have a client-intake form I was provided with some interactivity (text repsonse boxes). I'd like to add additional inputs to other questions on the form. For now just 'Save' is acceptable but a 'next step' would be connecting to a spreadsheet or db table to store values too.

Under each one radio buttons for 1-10 (mouse over shows #)
List of Major Emotions
Can click on any to choose (highlight yellow background) on or off
