Modify Excel Sheet or create new one

I would like to create an excel spreadsheet to
manage and follow patient complaints.

I have attached an inspiration for design. This
is the spreadsheet we use for scheduling our
patients

I would like Patient Complaint spreadsheet to
be similar, but will need modification to the
columns and dropdown options. The Tracker
will also need to be modified
