Microsoft Word, Macros, Form

I am looking for someone to create a form that is able to be filled using an excel so that multiple different copies of the form letter can be generated using a Macro. I have the form, the areas that need to be populated using the excel, and the Macro. I just keep messing up putting the form together! If there is a better more creative solution, I would also be open to that.
