• Group first checklist to the comment on next column
• Add checkbox (yes/no)(yes will be green and no red)to each checklist column with the checklist beside the checklist description. Layout Description first then checklist. Checkbox title issue.If checkbox is yes comments box will drop down underneath.
• Bottom of list Use Attachment where you can add camera pictures .
• Button to submit back to excel with message submitted
Then after submit the form be refreshed form new submitting
