Map PDF Form Inputs to Excel

I need assistance with a manual input task. The task involves mapping/transcribing data from PDF forms into an Excel workbook. I will provide the pdfs. The pdfs are fillable forms. Each input has a number assigned to it in blue in the pdf. I need you to populate a excel workbook where the caption/label of each input is mapped to the number of the input. Each tab in the excel workbook corresponds to one file. There are 4 different file types and 6 of each for a total of 24 pdfs that have to be mapped. Example files are attached, with a excel workbook that has a few inputs mapped.
