Lead tracker

I would like to have an excel spreadsheet that will let me have tabs where I can enter in my weekly leads and the information for each. I would like that template so I can start a new one each week. I want all of the data from the weekly tabs to be consolidated on one main page that tracks closed percent, conversation rate, active leads, cold leads
