Lead form that updates excel

I need a lead form with specific employee name in the url. When the user fills the form, the data is added to google sheets or downloadable in excel format. The form will automatically log the employee name when the lead form is accepted. Provision to add new employees will be a plus. Please start your bid with siliconoasis and you'll be given priority.
