A Jotform Simplification

A simplification is needed by adding an index where all sports and events will be displayed,
Today the user is selecting a sport and is jumping to the equivalent sport page,
so for each sport there is a condition on the form that send him to the appropriate page where he selects the corresponding event,
this is very nice but makes the form very heavy as it has 293 fields
there he selects the events and finaly confirms the data, This results in an output of 293 fields.
What is needed is to create a index form where the user will see the events he will particiapate,
and then fill out just the event id ,
the output of jotform is an excell sheet , however we need some rearrangement of the excell to map the structure of the attached EXCELL.
