Inventory database needed to convert existing Excel Inventory - Office 365 Sharepoint

I would like to create a database to be able to be used online. We have an existing excel file to be converted. I need a form to be filled out to add the inventory and have a list that is searchable by field, as well as the option to query by category and exporting results to file formats such as excel, choosing which fields to display in the query. PLEASE NOTE - We want to be able to use this online, either VIA Sharepoint - Optional for AZURE as well
