Interactive Bid Proposal Template

Hi! I need a template that has check boxes for a list of options our client can choose from, that also has a monetary amount attached to it. This is focused around Construction / BIM. For example, if our bid is $5,500 for the baseline, they could have option a for a 25% markup, option b for a 15% mark up, etc. When they check the box, I would like for it to update the total amount so they can see what their options look like! My original bid template is a word document and it is attached. You can see we have the options now only as asterisks as text, it is not interactive and they never tell us or read it.
