Help create and tutor in creating a google medical examination check list in excel and google sheet that outputs two separate paragraphs of text

I am looking to create an excel sheet (as initial proof of concept) and then google sheet (to increase functionality) to co-ordinate the capture and reporting of values in a medical (ophthalmic) examination of patients.
There are multiple parameters (input heading) recorded for each patient such as
Intra-ocular pressure
Tear production
Comfort levels
Fundic clarity
globe movements
blinking ability
pupil size
corneal appearance etc
Dependent on the input heading, the input may be a yes/no value, a number, a body of text, a choice from drop down box, a drop down box contingent on an initial selection.
Having inputted / selected values for each clinical parameter into the spreadsheet, I require two reports to be automatically generated in an output box that can be cut and pasted into another programme. It is the output text that defines the success of the project. Basically the reporting process will turn the individual inputs from the ‘checklist’ into a coherent and readable text in two formats. One will be an abbreviated text for entry into clinical notes and one will be a more narrative report.
For example a results box could have a default setting such as 'normal' which can then be overwritten to describe an abnormality. Some input boxes left blank would not appear in the final report. I have listed in the attached spreadsheet a mockset up of how the sheet might look. For clarity, each clinical parameter is hyperlinked to an individual page where the input options and the output text required is listed for that parameter. I would ideally keep this format so that if I need to alter output text as the project develops then it will be easy to go to the individual page for that parameter and make changes.
Ideally I would like to work with an expert who is prepared to teach in the development of this google sheet so I can be a close to self sufficient in the future if I need to add more parameters.
I foresee this as being a step by step process as I describe my needs with the project winner, and evolving the excel / google document creation to a point where it is clinically useful whilst I learn the process behind the sheets creation.
I would suggest that the first task will be to create demo sheet (with resulting output texts) dealing with a single patient type (cataract patient) where fewer parameters are collected. Once this is functional, the project will be widen to include a broader array of clinical inputs as per the attached spreadsheet.
