Google Spreadsheet entry needs to printed to a form and sent to a printer or PDF/Email

I have a paper sheet (DINA4) that needs to be filled with some personal data. I will have this data in a google spreadsheet. Some fields are individual as (Name, Adress, Phone number, gender and two more options to be marked....they all come from the google spreadsheet), two fields are static and one field gets the print date and print time. The attached picture shows part of the form where the data from the spreadsheet needs to be placed.
The blank DINA4 form will be in the printer and once there is a new entry (row) in the google spreadsheet table detected this entry should be printed automatically. Another option is to create a PDF and send it to an email account. We need an option to print selected rows/entries from the table a second time if there has been a problem with the printer or the data or whatever.
Long story short....right now we fill out the form with a pen and for future we will have the customer register online in advance and that data is filled to the google spreadsheet.
