Google Form based on variable from Google Sheet

Create an employee reporting system


We need a Task updation google form having following questions based on variable questions form Excel File (sheet named KRA & KPI)

- [ ] Name - Take from the Sheet “KRA & KPI " Column A
- [ ] Date (Compulsory in date format)
- [ ] KRA - Task (Dropdown from filtered database based on selection of name) - Take from Sheet “KRA and KPI " Column B
- [ ] Status - Dropdown (WIP / Pending / Completed)
- [ ] KPI - Quantity (No. Of Transactions / Events / Calls / Order Value etc.) - Take from Sheet “ KRA and KPI " Column C
- [ ] KPI - Quality (in Percentage terms - Accuracy / connected / Quality / Chance of conversion) - Take from Sheet “KRA and KPI " Column D
- [ ] KPI - Time (Time taken to do above task) - Take from Sheet “KRA and KPI " Column E
- [ ] KPI - Cost (Amount of cost involved to perform the above task) - Take from Sheet “KRA and KPI " Column F
- [ ] Remarks (optional)
- [ ] Attachments (Optional)

A link will be shared to each of our employees where a person can type the name of himself, and the questions of that person only will be shown to them
There should be an option to input the questions from an excel spreadsheet like their name, date, work to be done, etc.
