GEMBA Data Collection PowerApp

I am seeking a PowerApp developer to create a GEMBA PowerApp primarily aimed at data collection with a specific focus on task tracking.
"I want to build a responsive PowerApp for GEMBA observation. The app will start with a branded welcome screen that shows a logo and a New GEMBA Observation button.
When the user clicks that button, they go to a camera screen where they take a photo. The photo is then saved to SharePoint via a Power Automate flow that returns a link to PowerApps.
After the photo is taken, the user is taken to an entry screen where they fill in the observation details (description, 5S/Safety checkboxes, AssignedTo, Due Date, Status).
When they click Submit, the observation is saved to an Excel file stored in OneDrive, using a table named GembaTable.
The Excel and SharePoint setup are already done, and the Power Automate flow is named SaveGembaPhoto. All I need is for you to build this logic into a clean, responsive PowerApp."
