Fixing google spreadsheet / excel

Hi,
Most of the project is already finished, yet we need your help with our final page.
We created a google survey form, but the final page needs to be automatized.
When the survey is filled in, the subject name appears automatically with a score per module. This works fine.
What we then do is copy that score to another page to get it as a percentage. However we currently do this part manually, which is costing time and effort + room for error.
Task 1: Once a new name appears on page 3, gemiddeldes after the survey has been filled in, it needs to be automated to page 4 (uitkomsten) as a separate table with the text.
Task 2: The percentages should be more visual, fancier. We've added an example that we've found online.
