Excel - submit data from one sheet to another

Please review the attachment. A manual example of the functionality I would like is attached.
Instructions:
Preserve the formatting of "For Daily Use" tab. Make the "Submit" and "Reset" buttons functional.
Route "Submit" button to "Staffing History" tab so that all information from "For Daily Use" filters into the rows on the "Staffing History" tab.
Make the "Reset" button clear all data in the white dotted line cells, but not the data in the other cells.
