Excel data entry form in Excel with VBA code

Hi I have a requirement for an excel data entry form that needs to work in EXCEL 2010 . You can use simple VBA coding . This is something like a survey form with yes ,no, not applicable answers and a text box and a picture uploading option. One day time frame.
