Excel VBA 'tool' for generating forms w/ prefilled content

I have an older VBA ion excel that no longer works.
Whenever we are setting up a testing manual for a new project (quality control inspections), we compile and customize blank reports to be used in the field. These reports have a lot of common information (project name, project number, equipment type, specification number, etc.). We use to have a dashboard in Excel that would auto generate and customize these forms to eliminate the tedious work. This tool doesn't work as it has not been updated to work with Windows 10, cloud servers, etc. This leaves us having to manually enter this repeated data in all our installation and test forms. Mind numbing cntrl+C and cntrl+V...
We think that a lot of this can be accomplished through visual basic programming through excel (or alike), custom applications etc.
Would be nice to either revive this and 'fix' the programming or rewrite in newer format and clean out the redundant parts of the program.
Doesnt have to be pretty, just has to work properly. The older excel file worked great, had a few redundant and unnecessary background VBA. But had the right layout and format. Screenshot of the excel layout in the attached PDF.
