Excel VBA Mail Merge Template Fix Debug

I am looking for someone to fix a debug issue I'm having with an Excel VBA mail merge template. I am using Office 365 and getting error messages during the merge process. I need help debugging the code and would appreciate any assistance available. When using the Data on the evaluations tab after hitting the merge button it should show up as in the picture titled correct but shows up as the picture titled not correct. Ill provide workbook and password when chosen.
