Excel Trend Line Graph

Want to create a trend line report over time for people to track their lab test results. Need to track dates of events such dates of vaccination against dates if lab test results and its values. Template needs to dynamic so you can continue to add dates of vaccination against dates of lab results and its values. Important to be able to able to display visually to see trends.
