Excel Sheet

I want to create an excel macro enabled document file where we can key in the data in specific fields and a document is created pertaining to the data entered. Data should be as per the requirement as provided in the word file. The excel sheet shall be in designed in a way that once entering the data excel will select the data from database and fill in the relevant sheet to take print. Some data may be prefilled and some would be needed to entered. I am sharing an excel sheet which I prepared for reference and formats in which data is sought.
