Excel Scoring Matrix

We would like to create an excel document which we can populate with information in a scoring matrix of 10 key skills an employee does the scoring will be 1-5 and we would like this to be repulated over 12 weeks, with a overarching graph tracking there performance which is data taken to the 12 tabs of the weeks?
