Excel Formula Help

Hello everyone,
I am struggling to get the right formula. Please download the example excel sheet.
Basically, you will see a list of names and companies in range A4:D10.
What I need the sheet to do is enter a name in B2 and the sheet produce a list, shown in G4:J5.
I just cant get it to work! The Name is the unique identifier. Now I need someone good with excel to solve.
