Excel Form Creation

I need a skilled Excel expert to create a form for data collection concerning our project design plans. We have a word version (attached), but we would like to improve upon it. I would like Sections 1 through 3 to be auto filled by a GUI box where the user types the entries into a dialogue box.
The date sections should tie to a calendar to pick dates.
