Dynamic PDF Form

I am in need of a dynamic PDF form that can be filled out by users with certain data-driven components. I am seeking a skilled freelancer to create this custom form. The selected freelancer should have the necessary experience to handle this project. The desired outcome is a finished PDF form that includes the requested data-driven components, company branded and is in a professional and easy to use format. I attach a data structure overview showing the required calculations that need to populate to the PDF form and also a rough layout of finished calculations. Once the supplier has been selected I can share the full design brief and excel data sheet.
