Data entry lender list

I have a pdf document that contains a list of bridge lenders.  I would like to have this list placed into an excel so that i may upload to a CRM.  The data starts on page 16 of the attached  file with 3650 REIT as the first contact.

Below are the list of fields needed in excel:

Company Name
Contact Name (sometimes there are multiple contacts)
Contact phone number
Contact emails
Size
Senior
Subord
Max LTV
Term
Rate/Type
Notes
