Data-entry form for .tsv tables (to be use in Excel)

I need a convenient data-entry form to work with a tsv table in Excel. Tables are generated elsewhere and include words which need to be annotated using a pre-defined set of values. I attach technical specifications and a sample table (saved as Excel workbook to keep the formation for the purpose of this message; original tables will be in the tsv format).
