Creating Formulas in Spreadsheet

I am needing assistance to have someone help me create the proper equations in google sheets that will allow me to pull results for screening. I have attached a the spreadsheet which outlines all the specific requirements of what I am needing. Below is a just quick sample of this to give a better idea of what I am trying to build.

Guideline
Cervical Cancer: Screening If greater than 20 & female
Colorectal Cancer: Screening If greater than 44
Breast Cancer: Screening If greater than 39 & less than 76 & Female
