Create an excel form to record customer requests

I'm looking for a freelance employee to help me create an Excel template for storing customer orders. The template needs to record customer order details such as name, address, and the requested product, and the products should be displayed with images. I have sketched an idea for the order entry interface; you can take a look at the attached file with this request. I'm open to suggestions regarding the format or design. This template will be used by only one user.".
