Covert a word document to a dynamic survey for easy data analysis

I am looking for a freelancer who can help me convert a word document into a dynamic survey. The purpose of the survey is to gather market research data. I prefer to use Google Forms as the platform for the survey. At this moment, I am not sure if I will need help with analyzing the data after the survey is complete.
