Copy data from PDF to Excel/csv

I am looking for a freelancer who can help me copy data from a text-based PDF file into an Excel or CSV format.

Number of pages in the PDF file: 18
Formatting requirements for the Excel/CSV file:
One sheet, only the state name in the first column.
Keep the column headers as they are in the PDF.
Put a 1 in any cell with a checkbox and a 0 in empty cells.
Omit text from the cells.
Ideal skills and experience for the job:
- Proficiency in data entry and working with PDFs
- Familiarity with Excel or CSV files and ability to accurately copy data
- It may be possible to automate the PDF to CSV conversion.
