Converting spreadsheet data to Google form quizzes
Budget: $30 – $250 USD
I am the chair of the Extensive Reading Foundation. We have data for around 1000 quizzes on a flat spreadsheet. Each quiz has just three questions with 4 choices, with the correct choice always in the first field of each item.

What we need is an automated way to create separate quizzes for each item that would look like this:

Once this is accomplished, a second step will be to allow students to self register, take their choice of quizzes and see their score on their own spreadsheet. Right now, I am only interested in a quotation for the first step.
