Converting Excel Spreadsheet to Google Forms

Looking to convert a Microsoft Excel spreadsheet into Google Forms. There is a set of 80 questions each, questions has an answer range of 1-5. Questions are grouped together based on the category to come up with the total for the category. Would like forms to generate a chart at the end of the questionnaire. Require fields: all questions to be answered, required name, and email address: Google Form/Survey will be hosted (posted) on the church’s website.
