Convert a names and address list from PDF to Excel file

Pull names and addresses out of a PDF of printed pages and put them into an Excel spreadsheet. The Excel file should have column for phone number, Last name, first name, initial, email address, street address, city, state, postal code. Please look at the PDF attached to see the format they are in before bidding on this job. Please also look at the example Excel file which shows the first two contacts.
