Convert MS Word Data to MS Excel - Data Entry

Hi there
I have 83 MS Word files, each file is typically between 1 and 3 pages, some may be a bit longer. These files have transaction entries.
We need to move these transactions to following Excel format. Each transaction will be one new line item.

I have made some entries as an example. You also need to provide document reference in Column I.
