Convert 12 PDF documents into another format for Quickbooks

I have 12 documents that are in pdf format and I need them converted to one of the following formats to be able to upload into QuickBooks: CSV, .QFX, .QBO, .OFX, or .TXT format. I can only have 3 columns after formatting which include, date, memo and amount spent. These are pdfs of bank statements. I have attached an example of what I need formatted. I need these done ASAP.
