Company excel database & reports

Im looking for someone to create a few business spreadsheets for me.

Initially im looking for an Employee Database.

I need something that is easily fillable (form style entry), and customizable in terms of the employee data we gather.

Also a comprehensive dashboard is a must.

And lastly I would like the data to be uploadable to Power BI.

The following list would be the data headings for what we'd be collecting (capital letters are section headings, lower case are column IDs within that sectoin)...
