Coding a excel document in a Powerpoint format

I want to code an all in one document. I currently use an excel questionnaire which based on the clients answers creates a graph. I then copy and paste the graph onto a Powerpoint slide, add notes to this and send it to the client. This is very manual and is not very aesthetically pleasing.

I am looking for a way to have the questionnaire straight on the powerpoint document or similar which when completed automatically created the graph like the one on the attached PDF. If this is something that is possible I am happy to share the excel document showing the logic behind the calculations.
