Build Excel macro to take spreadsheet and output text

I have a spreadsheet with 5 columns. 4 of the columns are various levels of headings and the 5th is the information. The 4 columns are already sorted in either alphabetical order or a order that I specify.

I want to convert all the rows into information under the right headings without repeating the headings and output them into plaintext. You only need to create a macro that takes the Excel file and creates the output text file.
