Automated Template response using Mail Merge

The company I work for is doing a project on Burnout Assessment and we require some automation from with the use of Mail Merge.
A client fills out a questionnaire and then an email with the appropriate responses is generated. From this email, we currently enter in the data into a Microsoft Word template to generate a form then email it to the client. We wish to automate this process using Mail Merge, so that the information that is generated from the Email is auto-filled into the template into the appropriate sections, then forwarded as a pdf file to the client who completed the form.
Whether this can be done by converting the information from the email into an excel file or directly from the email, we wish to just automate this entire process.
