Automate filling in fillable in PDF's

I have a PDF fillable form to fill out that requires input of the same information several times (name, address, id number, etc). I attach that form. I need someone that can set up an easy database or something similar to automatically fill in this form. For instance I may have to fill in the form as many as 8 different times with same name, address, identification number and only a few numbers changes in the actual form. some of the information will come from the spreadsheet we have been developing. Attached is a sample form we would be working with.
