Adding QR generation to existing automated excel process

We're looking to edit an existing automated process created years back. Youll have to see the attachments to see the correlation below.

We would like to generate a QR codes containing the information below to then be printed on the LP and MP cards (as seen in the qr-vba-project-final.jpg) attachment.
