Simple Excel Data Entry Sheet

I need an Excel spreadsheet designed to automatically calculate weekly payments for a Doctor based on the number of patients seen and the type/duration of their treatments.

Requirements:

Create two sheets:

Services & Pricing – A table listing service types, prices, parking, and marketing fees.

Weekly Payment – A table to enter patient details, service type, and treatment count.


Add formulas to calculate the total per patient and the total weekly payment.

Make it easy to update prices in the Services & Pricing sheet without breaking the formulas.

The spreadsheet should be clean, easy to read, and error-proof.
