Building a MS Access Database

I’m trying to build a database that register patients’ vaccination in 3 vaccination spots. I thought that I was able to do that by myself but my access skills are gone.
This is for a nonprofit purpose, only a college who wants to help to get rid of a mountain of paper.

There are three vaccines that the patient has to agree.
The vaccination procedure will start only after 4 weeks from the registration date
