Prepopulate contract for electronic signing from from CSV file

Hi Awesome Freelancers,
I have a problem where I need a tool to assist me prepopulate finance contracts with customer details that we receive in a CSV file.

An example of the type of CSV file we receive is attached and the contract we need to generate and send for electronic signing is also attached. I am open to ideas on what electronic signing tool we use also.

We do not recieve detail of the equipment (make, model, serial number) as well as pricing so ideally if there was a way we could take the CSV file, have the data transferred to another file or system and there we input the equipment details (description, make, model, serial and price) and other info such as monthly payment, term etc and then the system can generate the contract and send to client for sigining.

I would be interested to learn if there is some way of making this work.
