Interactive Excel Performance Appraisal Form

I'm looking for an interactive Excel form for performance appraisal. The form must include:

- Automated scoring
- Dropdown selections
- Clickable buttons (e.g., option buttons for rating)
- Macro-based features for appraiser comments

It should be compatible with both Windows and Mac Excel. Ideally, you would have experience in Excel automation and form design.
