FooEvents Ticket Theme Development

I need a theme for FooEvents based on my pre-printed ticket design. The tickets are in a 210 x 100 mm format.
Requirements:
- Left-hand section with empty fields and labels should print corresponding values upon ticket booking.
- You will work from my provided design in an alternative graphic file format.
- The design includes all necessary elements.
