Guidelines:
You are part of an interprofessional community-based team providing healthcare services. Tangata whaiora (a
person seeking health) has been referred to the service, and your supervisor believes their case would be
valuable for your professional development portfolio. Your supervisor has offered to assist you in meeting the
requirements and will provide feedback on the information you plan to present. They have indicated that it will
be important to highlight occupation-based theory and practice.

Use national and international evidence-based literature throughout your assessment
Ensure you integrate evidence throughout your assessment. All literature and resources must be appropriately
acknowledged using APA 7th edition referencing. You are expected to include a minimum of 12 relevant and
reliable sources, published within the last 10 years, to support and inform your work.

Using the International Classification of Function (ICF) undertake a Health Condition Analysis (HCA) in
relation to the given health condition e.g., aetiology, epidemiology, risk factors, signs and symptoms, onset,
course, and prognosis. Include possible activity limitations and/or participation restrictions. (approx. 400
words).
Explore and discuss the experiences, and potential implications for this population (Parkinson’s or related
conditions) in relation to occupational justice (approx. 350 words).
Justify why occupational therapy input is appropriate for the population (approx. 100 words).
2. Occupation-based practice (using the case information) (approx. 850 words).
Discuss how key aspects of the health condition could impact tangata whaiora participation in meaningful
occupations and discuss in relation to occupational science concepts. (approx. 200 words).
Using professional reasoning discuss how the chosen occupation-based model, and its associated process
will guide your occupation therapy practice with tangata whaiora, to establish needs. Then identify two
potential occupational needs. (approx. 300 words).
Identify one standardised and one non standardised assessment and justify the use of each of the
assessments with tangata whaiora. (approx.250 words).
