Linguist needed, healthcare area

We need a linguist to review a questionnaire that will be read to consumers. The document needs to be checked for clarity and intent. We'd brief the linguist, tell them what we are after, and then have the linguist provide recommendations. At this point we cannot upload the contents, but can do so after discussion.
