I want to design a nursing multi tool pen.

I would like the pen to a be a four clicker with (black ink, red ink, a permanent marker and a white board marker) I would still like the pen to be the size of a four click pen. I would like the pen to have a neurological torch on the end. I would also like the pen to have a small tape measure built in under the clip (10cm) I would like the pen to be metal and be able to be custom designed to suit the buyer. I would also like the pen to have refillable option for cost effectiveness
